Item CARCINOMA MIOEPITELIAL ORIGINADO EN UN ADENOMIOEPITELIOMA UN REPORTE DE CASO(Ciencia Latina Revista Cientifica, Ciudad de Mexico, 2025-06-21) Andres Augusto Reyes Agudelo.; Juanita Panqueba SalgadoABSTRACT Objective: To describe the case of a patient with myoepithelial carcinoma originating from an adenomyoepithelioma, a condition that is extremely rare (less than 2%) among neoplasms originating in salivary glands, but occurring in the breast. Additionally, a critical discussion of the evidence supporting the management of this uncommon pathology is provided. Case Presentation: The case of an 80-year-old female patient with breast pain, a palpable mass, and bloody nipple discharge is presented. The biopsy revealed a myoepithelial carcinoma originating from an adenomyoepithelioma, with a 3-mm invasive component. Oncoplastic surgery with central quadrantectomy showed clear margins, and sentinel lymph node biopsy was negative. Given the clinical stage of T1N0M0, close follow-up was recommended without adjuvant radiotherapy. Conclusion: Adenomyoepithelioma (AM) is a rare breast tumor, typically benign, but with potential for malignant transformation, especially in triple-negative cases. We present the case of an 80-year-old patient with malignant AM, diagnosed through histopathological and immunohistochemical findings. Despite imaging findings suggestive of ductal carcinoma, the biopsy revealed an epithelium-myoepithelium pattern, confirming malignancy. Treatment included surgery with clear margins and adjuvant radiotherapy, with no recurrence during follow-up. This case highlights the importance of considering AM in the differential diagnosis of breast lesions in postmenopausal women and the utility of immunohistochemistry in its diagnosis.
